The first 25 selected families will receive a structured medication safety review designed to help them better understand whether multiple medications, recent changes, possible interactions, or expected side effects could be contributing to what they are seeing.

Tell us about your loved one, their medications, and what is worrying you most.
Step 2: Receive a review
If selected, your family receives a structured review in plain English with watch items and questions to raise with a doctor or pharmacist.
Step 3: Give feedback
In return, we ask for honest feedback and an optional testimonial if you found the review genuinely helpful.
